Permission to Bring Dog(s) for Emergency Veterinary Care:

I grant permission to Danielle R. Pellicci, BLKFTK9 d.b.a. BlackFoot Kennels and/or members of her staff to transport said dog for veterinary care if it is necessary to do so. It is understood that emergency care may not necessarily be issued by the owner’s regular veterinarian. By signing, the owner grants permission to Danielle R. Pellicci and her staff to make decisions about the immediate health care of said dog in an emergency situation. It is agreed that all veterinarian expenses will be reimbursed by the owner in the event that immediate costs must be paid at time of treatment.

Media Permission:

I grant permission to Danielle Rosalie Pellicci and her staff at BLKFTK9 LLC d.b.a. BlackFoot Kennels to use my dog's photographs and / or videos taken during training for educational and promotional purposes in any type of media.

General Liability:

I understand that enrolling in dog training may involve risks to myself, members of my family, or my dog. I assume all risks participating in dog training group classes or private lessons, or boarding with training for my dog conducted at Blackfoot Kennels. I will not hold Danielle Rosalie Pellicci, BLKFTK9 LLC d.b.a. BlackFoot Kennels or any assistants and instructors responsible in the event of injury to myself, my family members, or my dog. I agree to comply with the verbal and/or written instructions given to me from my training instructor(s) as it relates to me or my dog's abilities to safely participate in training. I also agree to assume and responsibility to any damages done to property, persons, or other dogs done by me or my dog's actions. I understand and agree that if my dog exhibits a serious aggression issue pertaining to other dogs or humans, it will be removed from training and no refund will be offered. I acknowledges that when enrolled in a training program, I grant Danielle Rosalie Pellicci d.b.a. BlackFoot Kennels and her instructors/staff to use any training equipment deemed appropriate to work with my dog. Examples include but are not limited to: such as martin age collars, slip collars, prong collars, and electronic collars. I acknowledge that these are tools that will only be used in a humane way by educated professionals. As lawful consideration for participating in any training program at BlackFoot Kennels, I, for myself, my heirs, next of kin, executors, administrators, legal representatives, successors and assigns (the “Releasing Party”), hereby waive, release, discharge and covenants not to sue and to indemnify, defend and hold harmless Danielle Rosalie Pellicci / BLKFTK9 LLC d.b.a. BlackFoot Kennels, her instructors and members, managers, agents, and employees (the “Released Party”) from any and all injuries, losses, claims and damages to any person or persons of any nature its directors, officers, employees, agents, sponsors, organizers, volunteers or other representatives, their successors and/or assigns, (hereinafter collectively the “Releases”) from any and all liability to the Undersigned for any loss, harm, damage, claim, injury or accident to the person or property of the Undersigned, whether caused by any ordinary negligent act or omission of the Releases while the Undersigned or his/her dog is engaged in or participating in dog training/boarding or using any trainer or business facilities in connection therewith.

Care and Maintenance of Dogs:

If dog is found to have fleas, ticks, or internal parasites, it will be removed from training until the dog has been treated, and free of infestation (at owner's expense). For any dog boarding at BlackFoot; We reccomend that you provide monthly flea and tick topical applicators in addition to *manditory* monthly heart worm preventative. Intestinal parasites are sometimes un-avoidable despite the cleanliness of an environment. If a dog is staying at BlackFoot Kennels for 6 months or longer, we recommend using a canine wormer once every 6 months. We feed and recommend either: Purina One dog Food or Purina Pro Plan Sport. We feed each based on their diary needs, performance requirements, activity levels, and body condition. We feed the dogs twice daily to avoid bloat and gastric upset & to ensure proper energy needs. All dogs enrolled in our board wityh training program are kept in a safe, covered run or outdoor/indoor kennel. Some dogs are crated on occasion for various reasons in our climate controlled in-door “dog room.” Each boarding space has constant fresh water available. Out-door runs each have a raised cot style dog bed and we provide the dogs safe chew toys to keep them occupied when they are not training or exercising. We do our best to maintain a sanitary and dry kennel environment. Someone is on sight nearly 24 hours a day with the dogs. This allows for plenty of time in the airing yard to play and eliminate. In case of severe weather we reserve the right to move dogs indoors to be crated.

Transportation and Environmental Exposure:

The owners acknowledge that said dog may be transported to training areas off site from the BlackFoot Kennels location. Owner acknowledges that said dog will have exposure to gun fire, swimming water, and various terrains that could be dangerous for dogs. At BlackFoot Kennels we never want a dog injured during their stay with us. We vow to do our very best to keep all dogs safe in every way. Due to the nature training environments, and general dog activity/ behavior we must take into consideration possible accidental situations. Please note that by signing this waiver you are agreeing to not seek any legal or financial action against Danielle R. Pellicci, Steven Pellicci, or any BlackFoot Kennel employee in the case of death or injury of your dog from accidents including but not limited to: Snake bites, alligator attack, heat-stroke, death or injury from automobile, severe allergic reactions, or gastric dilatation/volvulus (bloat). All veterinary expenses are to be paid for by owner.

Payment

For Private Lessons, group classes, workshops, and AKC events: Payment in full is due before or at time of training (see individiual program details for payment options). For board with training: A non refundable deposit is due to hold a spot on the board and train schedule. An installment payment will be due at time of drop off and balance will be due before or at time of pick up. Please see the web site www.BlackFootKennels.com for an updated list of fees. Your dog will not be returned without appropriate compensation for the time in training. The dog will continue to be trained for up to one month after an invoice had been issued. Failure to pay after this 30 day period will result in: a) cease training the dog and b) sale or re-homing of the dog.

Training Guarantee

Your dog's training will only be effective when maintained and practiced with consistency on a regular basis. If you are not satisfied with your dog’s training progress it must addressed prior to taking the dog home. Once any dog enrolled in boarding with training leaves the property, it is up to the owner to practice in order to sustain the dog's training. Similarly, any person participating with their dog or puppy in group classes or private lessons must also work on the assigned homework they learn during lessons in order to train the dog. For this reason, we do not guarantee any training and all payments are non refundable.
